WHY?
Ketamine is one of the most effective treatments in the world for pain and mental health conditions. It is listed in the World Health Organization’s list of necessary medications. Ketamine was used in the military setting for decades for anesthetic purposes and used for anesthesia prior to surgery as well. Research has shown that it is very effective in reducing depressive symptoms with severe depression.
WHAT?
Ketamine is an NMDA receptor antagonist. This means it is able to block certain receptors in our brain, creating the effects of a dissociative drug that works as a psychedelic agent. It is in the same class of drugs as psilocybin (magic mushrooms), LSD, MDMA, DMT, and ayahuasca. Ketamine is given most commonly through intravenous routes as well as intramuscular injections. These routes provide the most available amount of the drug in your body's circulation and tend to be the most effective. One can also medicate with Ketamine through an inhalation route with a nasal spray of Esketamine (a specific subtype of Ketamine). Oral Ketamine can come in the form of lozenges and troches. Each route of administration has specific indications, benefits, and medical necessity.
HOW?
Ketamine is short acting, so it can be given in a therapy session of less than an hour. Although you cannot drive home from this treatment, you can have a fairly normal day, as the effect wears off within a few hours. The mental health effect, however, lasts for quite a while, extending in time with each successive treatment. It is suggested to have an induction series of 6 treatments to create more permanent change, with an availability of “reminder” doses thereafter, as needed. This could be monthly, quarterly, or biannually.
WHEN?
It is best to have treatment on an empty stomach for at least four hours with only water up to 2 hours before. It is advised that the sessions be at a relaxing time, with a clear schedule for yourself to have the optimal treatment experience. This session should be all about you and no other commitments. Self-care is not limited to just receiving the treatments but also includes the environment in which we receive treatment. FUTURE SOLUTIONS
It is my hope to build a practice with an array of psychedelics used in concert with ketamine inductions, psilocybin micro-dosing for maintenance, and MDMA for therapy. MDMA has just graduated from its third stage clinical trials, proving to be greater than 80% effective in treating PTSD. These patients involved in the trials showed decreased symptoms of PTSD, and 80% are symptom free after the three month course.
